How to register
You can now register for the on campus Medical Centre online!

Print out and complete these three forms:
- Registration Form (PDF)
- Health Questionnaire (PDF)
- Audit C Alcohol (PDF).
Your NHS number can be obtained from your home GP.
Please return the completed forms to us:
- by email to studentmedicalcentre@nhs.net
- or by hand to the Medical Centre (find us on the Campus maps)
- or by post to:
Student Medical Centre
Old Court
Froebel College
Roehampton Lane
London SW15 5PJ.
Once you have registered, please keep us informed of any change in the information you have provided.
If you have a significant medical condition (eg diabetes, asthma, epilepsy) or you are on regular medication, we advise you to come to the Medical Centre to update your GP details as soon as possible. Please bring sufficient supplies of your medication to last for the first month or so at University: this includes the oral contraceptive pill.
Immunisations
All students should be up to date with their primary immunisations of:
- Diptheria, Tetanus and Polio (five doses)
- MMR (two doses)
- Meningitis C.
If you are unsure of the above, please contact your home GP.
